Hydroxychloroquine is a synthetic antimalarial drug and has been the clinical treatment of rheumatic diseases irreplaceable first-line drugs. Hydroxychloroquine has been suggested to have beneficial effects on lipids and insulin sensitivity, which may contribute in lowering high cardiovascular risk in SLE patients. Hydroxychloroquine, an alkylated 4-aminoquinoline, is a hydroxylated form of chloroquine and has a biological mechanism of action similar to chloroquine’s. Hydroxychloroquine was originally developed as a safer alternative to chloroquine for the treatment of malaria and was later found to have antirheumatic and anti-inflammatory uses. The purpose of this study is to determine the short-term effects of the antimalarial medication, hydroxychloroquine HCQ, compared with placebo using frequently sampled intravenous glucose tolerance testing FSIGTT methodology to study changes in insulin secretion and glucose tolerance in subjects at risk for developing Type 2 diabetes.

Hydroxychloroquine is used to treat lupus erythematosus and rheumatoid arthritis. It isn’t fully understood how this drug works to treat lupus erythematosus or rheumatoid arthritis. That means you may need to take it with other drugs. It treats malaria by killing the parasites that cause the disease.

Hydroxychloroquine has been shown to have a dose‐dependent insulin‐sparing effect in diabetic rats. 29 In addition, there are case reports of hypoglycaemia induced by hydroxychloroquine in non‐diabetic patients with rheumatoid arthritis. 30 In one study of patients with non‐insulin‐dependent type 2 diabetes controlled by diet alone, chloroquine at a dose of 250 mg four times daily decreased the metabolic clearance rate of insulin by 39%, increased fasting C‐peptide secretion by. A total of 2619 patients without previous kidney diseases were diagnosed with rheumatoid arthritis in the LHID2005 between January 1, 2000 and December 31, 2013 Figure 1. Of them, 1212 patients were hydroxychloroquine users, and 1407 were nonusers. Hydroxychloroquine is a prescription drug. It comes as an oral tablet. Hydroxychloroquine is available as the brand-name drug Plaquenil. It’s also available in a generic version.
